    Situation

    Hospital group with four sites, 1,600 nurses. Attrition up 18 % year over year. Previous surveys flagged 'workload' as the top theme — but no defensible intervention point.

    Approach

    Confidential 12-minute conversations in German, English, Romanian, Filipino and Croatian. Frame: the last four weeks, with dynamic probing on rota, leadership, equipment and recognition.

    Result

    Not 'workload', but rota fairness surfaces repeatedly as the top blocker — especially in houses with short-notice changes. 'Workload' was the symptom; planning uncertainty was the cause.

    Consequence

    Retention programme with clear rules for short-notice changes in two pilot houses. After six months, attrition in the pilot houses drops 9 percentage points versus control houses.
